Add authenticity token to request-form helper

Detailed description 
Due to changes in Edge, fbml are now considered 'verifiable request formats.' Meaning, if you have forgery protection
turned - your post requests will fail if you don't include the authenticity token.

Rather than turning off forgery protection for facebook, I've modified the request form helper to add the authenticity
token to the request parameters.
