patch to get Sphincter working with postgresql (and a fix for the initial index task)

Detailed description 
This patch gets rake sphincter:configure and rake sphincter:reconfigure to work if the user is using postgresql for
version 1.1.0.  In the spirit of Sphincter, no external edits are required...

It also contains a fix for rake sphincter:index so that it generates an index if no previous indices exist.
