Summary: warn user when two examples with the same name appear in the same example group

Initial Comment:
Right now if you do this:

describe Foo do
  it "should bar" do
    ...
  end

  it "should bar" do
    ...
  end
end

RSpec will run both examples and provide output like this:

Foo
- should bar
- should bar

Perhaps rspec should warn you or raise an error when you do this.
