I think you can use this little combo:
1. pdftops (and not pdf2ps if you want to keep you fonts embedded in the final pdf)
2. psjoin (perl script attached. I found it quite useful - author: Tom Sato => http://homepage3.nifty.com/tsato/)
3. ps2pdf or pstopdf
