[Instiki] Revisions

it's based on the amount of time past since the last edit. which is  
nice since there's no preview. usually how i know when i'm done  
making changes is when i stop making changes.

it also kicks off a new revision when a different user makes an edit,  
so if you want to force one you can just change your name.
